Post-Deactivation Feedback Form

A simple reflection tool to wrap up your time with a student!
When your time with a student comes to an end —whether they've graduated Year 12 or are simply happy with their abilities— it’s important to capture the progress you’ve made together. That’s where the Post-Deactivation Feedback Form comes in.
This form contains similar diagnostic questions to the Trial Session form - covering your student’s performance & feelings regarding their subject, but now at the end of your time together. You, their parents, and us will be able to see how much the student’s grades, motivation and outlook improved over time!

What's in the Form?
The form is split into three sections. The first half you complete with your student, during the final lesson. You’ll need their input on a few self-reflecting questions! The second half you complete by yourself.
Part 1: Crunching the numbers 📈
Here, you’ll record some cool stats like how many lessons you completed, and how long you were working together. You’ll be surprised how time flies!
Then, you’ll be directed to chat to your student about the highest grade they received all year (their Personal Best!), their average grade, and their current confidence level.
Part 2: Goals and Achievements 🏆
Here you’ll be prompted to consider:
- The student’s biggest areas of improvement (academically, behaviourally, or in terms of mindset)
- Any final thoughts or guidance you’d like to share. For example:
- If a student is leaving because they now feel confident in the subject, you can encourage them to choose it during subject selection for senior years.
- If they’re heading into Year 12, you could offer your encouragement and a few strategic tips for maintaining balance and motivation.
- For younger students, something to keep nurturing their love of learning can go a long way.
Part 3: Final Comments ✍️
This is your space to make a closing statement about the time you shared with the student. There’s no solid rulebook for this section! Be personal, thoughtful and sum up your journey. We’re all about being ‘genius best friends’, so feel free to change each message between your students. It might include:
- What you’ve appreciated about working with them
- A highlight from your sessions
- A note of encouragement or congratulations
Why the Form Matters?
When you're working with a student week after week, focused on assessments, assignments, and tricky concepts, it’s easy to lose sight of how far they’ve come. This form gives you—and them—a chance to zoom out and see the bigger picture of improvement.
It’s also:
- A powerful tool for you to see the rewarding results of your work in one final snapshot
- A helpful archive for us to better understand and advertise the calibre of tutoring you’ve delivered
- A concrete way to collect large-scale feedback about a student’s growth in grades, motivation, and outlook
We’ll also use this form to help evaluate patterns across different students and tutors, so your thoughtful reflections help shape the bigger picture of what tutoring success looks like here at KIS.
